What is a Support Broker and How Can Services Assist Your Path to the Best Life Possible?

Planning and directing supports and services to help you, or your loved one, live in a safe and healthy home environment, or actively in the community. Support Broker services can be short-term, time-limited, long-term, or on-going, to assist with specific goals. If utilizing IRIS program services, your IRIS Consultant will need to authorize support broker services on the Individual Support Services Plan (ISSP). IRIS Support Broker Services, after authorization from the IRIS Consultant, will be offered independent of any other plan services to maintain a conflict of interest policy.

IRIS Support Brokers, are cleared to serve participants by completing a caregiver and criminal background check.


Support Broker Education and Experience

  • BA in Human Services

  • State of Wisconsin Certified Social Worker

  • Evergreen Certified Dementia Care Specialist through Evergreen Certifications, LLC

  • Certified for Future Planning through The ARC (after April 1, 2021)

  • IRIS Consultant Employment Experience (2015 to 2018)

  • Former Special Needs (level 4) Foster Parent

  • Over Twenty Years of Human Services Work and Advocacy

  • Experienced with all ages across the life span

  • Personal Experience as a caregiver, parent, and legal guardian

  • Referrals to Financial Resources to Special Needs Trusts, ABLE Savings Accounts, or Financial Planning

  • Medicare/Medicaid Programs, as well as Support with Food Share Benefit Applications or Renewals

  • Recruiting Caregiver/Employees and Hiring Employees
